HOP Rack Card

The rack card offers a quick overview of HOP for prospective clients. It is a two-sided piece: one side in English and the other in Spanish.

HOP Brochure

This comprehensive brochure provides important information for prospective HOP participants, including eligibility criteria and a full list of services. It is available in English and Spanish. We have left space on the back of the brochure for you to provide your organization’s name and contact information.

HOP Services Flyer

This bilingual flyer is a guide to all HOP services and is suitable for clients interested in HOP or those already enrolled who may need additional services. One version directs clients to Impact Health for more information; the other has room for your contact information under “Learn More”.

HOP Wall Poster

The wall poster, available in English and Spanish, can be hung at your organization’s office or in frequented locations in your community.
