Impact Primary Care Network (IPCN) is a physician-led network in Western North Carolina (WNC). As a nonprofit, our clinically integrated network (CIN) is leading a new approach to delivering whole-person care. This approach prioritizes the needs of providers and their patients. IPCN’s providers understand our communities’ needs because they share them. We are deeply invested in our patients’ health and the health of our rural region.
IPCN offers a streamlined care management platform and high-quality care management services. All services leverage tools to address our patients’ health-related social needs. Our practices have access to a robust network of organizations delivering critical social services. Our relationship with Impact Health supports participation in Medicaid’s Healthy Opportunities Pilot and Medicare’s Making Care Primary initiative.
We are creating meaningful solutions to primary care challenges. These solutions help practices shift to value-based payment models while improving access to care.
