We encourage you to learn more about the Healthy Opportunities Pilot (HOP) and the services that may be available at no cost to your clients who are Medicaid beneficiaries. You can invite them to contact their care manager or Impact Health at 828-278.9900 to request a HOP eligibility screening.

Pilot-Eligible Services

NC Medicaid members with at least one (1) physical or behavioral health criteria and one (1) social risk factor living in our pilot region may qualify for one or more of the following services under HOP:

Housing

Essential utility set-up
Healthy home goods
Home accessibility and safety modifications
Home remediation services
Housing move-In support
Housing navigation, support and sustaining services
Inspection for housing safety and quality
One-time payment for security deposit and first month’s rent
Short-term post-hospitalization housing

Food

Diabetes prevention program
Evidence-based group nutrition classes
Food and nutrition access case management services
Fruit and vegetable prescription
Healthy food box (pick-up or home delivery)
Healthy meal (pick-up or home delivery)
Medically tailored home-delivered meal

Transportation

Reimbursement for health-related public and private transportation
Transportation for case management services

Toxic Stress Services

Evidence-Based Parenting Curriculum
Home Visiting Services

Cross-Domain

Holistic high-intensity enhanced case management
Linkages to health-related legal supports
Medical respite care
