Impact Health Network

Our Human Service Organization (HSO) Network is the heartbeat of the Healthy Opportunities Pilot and Impact Health. Every day, these organizations deliver services to help keep Medicaid Managed Care members healthier and reduce the need for clinical services.

Our HSOs help people secure and maintain safe housing; enjoy regular, healthy food and nutrition education; access health-related transportation; and avoid harmful interpersonal relationships. The organizations that provide these services contract with Impact Health to deliver them to eligible Medicaid members. In turn, they receive Medicaid reimbursements for every HOP service they deliver, which provides a dependable source of income for their community service work.
