Impact Health’s network members are the heart of the Healthy Opportunities Pilot (HOP). Our human service organizations (HSOs) connect NC Medicaid members to safe housing, nutritious food, and reliable transportation. They also support families in creating safe and healthy relationships. Together, our HSOs have delivered over 200,000 HOP services to eligible community members. Their efforts are helping pilot participants get and stay healthier.
