Impact Primary Care Network (IPCN) is a nonprofit organization led by primary care physicians across Western North Carolina (WNC), dedicated to working together to deliver comprehensive, high-quality care for our community.
Established by Impact Health in 2024, IPCN supports member physicians by regularly following up with their patients to ensure they have access to the resources needed to help optimize their health. Our nurse care managers guide patients through their care plans and medications. They also provide check-ins after hospital stays or ER visits to connect individuals with the right support for their ongoing healthcare needs.
IPCN helps patients access essential social services like nutritious food, safe housing, and transportation to medical appointments. We collaborate closely with their doctors and nurses to coordinate whole-person care and ensure every patient has access to the resources they need to support their wellbeing.
To empower our network physicians, IPCN offers high-quality care management and data analytics tools, supports quality improvement initiatives and value-based care, and negotiates contracts with health insurance payors.
