North Carolina’s Healthy Opportunities Pilot (HOP) helps eligible NC Medicaid members access nutritious food, stable housing, reliable transportation, and healthy relationships. By investing in evidence-based interventions, the goal is to improve health and lower healthcare costs.
Pilot services are provided by a network of community-based human service organizations (HSOs). In Western North Carolina, Impact Health leads a network of more than 60 HSOs serving 18 counties. Our HSOs are trusted community partners working hard to meet their communities’ needs. Many rely on grant funding and donations to provide services. Joining HOP allows network members to bill for services provided to pilot participants. HOP creates a reliable income stream that can support other programs and services.
Organizations that join our network become even better at serving their community. Impact Health builds members’ capacity to provide high-quality services. We offer ongoing support for delivering services, handling invoices, training staff, improving quality, and becoming financially sustainable.
Impact Health isn’t recruiting new member organizations right now. But we keep an eye on our network’s capacity and add organizations when needed. To qualify, groups must offer essential pilot services in Western North Carolina and be legal entities in North Carolina.
