If so, the Healthy Opportunities Pilot (HOP) could be a lifeline. HOP offers no-cost services to eligible NC Medicaid members to support their wellbeing. These services provide access to fresh food, safe housing, reliable transportation, and personal safety.

The HOP Toolkit can help you connect eligible community members to healthy opportunities. These resources explain who is eligible for pilot services. They also identify the services that are available to help people live their best, healthiest lives.
