The Healthy Opportunities Pilot (HOP) may be able to help you support patients whose care you manage. HOP provides no-cost services to eligible NC Medicaid members. These services provide access to fresh food, safe housing, reliable transportation, and personal safety.
The HOP Toolkit provides easy-to-use tools to help you communicate clearly about HOP with patients. These tools can help you determine their eligibility and explore pilot services that can aid them in improving their health and living their best lives.
