Are you currently enrolled
in NC Medicaid?

If so, the Healthy Opportunities Pilot (HOP) may be able to help you with many nonmedical services to improve your wellbeing at no cost to you. HOP can help you access fresh food, safe housing, reliable transportation, and healthy relationships. These things are as important as regular doctor’s visits to keep you and your family healthy.

To be eligible for HOP services,
you must:

Be enrolled in an eligible NC Medicaid health plan

Have a qualifying health condition like diabetes, heart disease or lasting mental illness

Have a qualifying social need, like being unable to access healthy food or safe housing

One-time security deposit and first month’s rent

Home accessibility and safety modifications

Transportation

Transportation to support health needs like the grocery store or fitness center

Reimbursement for gas mileage

Vehicle repairs

Personal Safety and Healthy Relationships

Evidence-based parenting classes

Home visiting services

Violence intervention services
